Age UK Wandsworth has launched a new service aimed at helping older residents across the borough live independently, safely, and with greater social connection. The Age Well Together programme replaces the charity’s previous Better at Home service and offers a streamlined package of support designed to prevent decline, reduce isolation, and promote wellbeing.
The service includes help transitioning home after hospital stays, regular online shopping orders, and befriending through phone calls and home visits. It also provides practical assistance with home adaptations, such as the supply and fitting of key safes, grab rails, and stair rails to reduce the risk of falls.
Residents, carers, and professionals can refer individuals to the service using a dedicated referral form, and the Age Well Together team can be contacted directly via awt@ageukwandsworth.org.uk or by phone on 020 8877 8940.
Age UK Wandsworth is the borough’s leading charity for older people, serving around 15,000 residents annually. Formerly known as Age Concern Wandsworth, the organisation has been active for over 60 years, evolving from a small volunteer-led initiative into a comprehensive support network offering everything from advice and advocacy to day centres, digital inclusion, and community engagement.
The charity operates from its main office on Old York Road, with additional outreach sites in Roehampton and a day centre on East Hill. It is independent of national Age UK funding and relies on local donations, grants, and partnerships to sustain its work.
